Read the summary of CAG\u2019s recommendation from the article to date below:<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><a href=\"https:\/\/www.cag-acg.org\/images\/publications\/CAG-COVID-19-Vaccines-in-IBD-Patients.pdf\">Click here for the full manuscript from CAG.<\/a><\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Current timeline of updates for the COVID-19 Vaccine<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>December 11<sup>th<\/sup>, 2020:&nbsp;<\/strong>&nbsp;the&nbsp;<a href=\"https:\/\/www.fda.gov\/emergency-preparedness-and-response\/coronavirus-disease-2019-covid-19\/covid-19-vaccines\" rel=\"sponsored nofollow\">Food and Drug Administration (FDA)<\/a>&nbsp;issued an Emergency Use Authorization (EUA) for Pfizer-BioNTech COVID-19 vaccine in persons aged 16 years and older for the prevention of COVID-19.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">This was then followed by a recommendation for the use of this vaccine by the&nbsp;<a href=\"https:\/\/www.cdc.gov\/vaccines\/hcp\/acip-recs\/index.html\">Centers for Disease Control and Prevention (CDC)<\/a>\u2019s advisory committee on Immunization Practices (ACIP).<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>December 18<sup>th<\/sup>, 2020:<\/strong>&nbsp;the FDA issued an emergency use authorization (EUA) for moderna COVID-19 Moderna vaccine in persons aged 18 years and older.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Is it safe for immunocompromised patients to receive the COVID-19 Vaccine?<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">The CDC then issued guidance on vaccination of special populations including immunocompromised persons:<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">&nbsp;\u201cPersons with HIV infection or other immunocompromising conditions, or who take immunosuppressive medications or therapies&nbsp;<a href=\"https:\/\/www.cdc.gov\/coronavirus\/2019-ncov\/need-extra-precautions\/people-with-medical-conditions.html\">might be at increased risk for severe COVID-19<\/a>. Data is not currently available to establish vaccine safety and efficacy in these groups. Persons with stable HIV infection were included in mRNA COVID-19 vaccine clinical trials, though data remain limited.&nbsp;<strong>Immunocompromised individuals may receive COVID-19 vaccination if they have no contraindications to vaccination.&nbsp;<\/strong>However, they should be counseled about the unknown vaccine safety profile and effectiveness in immunocompromised populations. As well as the potential for reduced immune responses and the need to continue to follow all&nbsp;<a href=\"https:\/\/www.cdc.gov\/coronavirus\/2019-ncov\/index.html\">current guidance<\/a>&nbsp;to protect themselves against COVID-19\u201d Source&nbsp;<a href=\"https:\/\/www.cdc.gov\/vaccines\/covid-19\/info-by-product\/clinical-considerations.html\">here.<\/a><\/p>\n\n\n<div class=\"wp-block-image\">\n<figure class=\"aligncenter size-full is-resized\"><img decoding=\"async\" src=\"data:image\/gif;base64,R0lGODlhAQABAIAAAAAAAP\/\/\/yH5BAEAAAAALAAAAAABAAEAAAIBRAA7\" data-src=\"https:\/\/cdhf.ca\/wp-content\/uploads\/2022\/07\/Is-it-safe-for-immunocompromised-patients-to-receive-the-COVID-19-Vaccine-.webp\" alt=\"COVID vaccine bottles\" class=\"wp-image-7428 lazyload\" width=\"512\" height=\"342\"\/><noscript><img decoding=\"async\" src=\"https:\/\/cdhf.ca\/wp-content\/uploads\/2022\/07\/Is-it-safe-for-immunocompromised-patients-to-receive-the-COVID-19-Vaccine-.webp\" alt=\"COVID vaccine bottles\" class=\"wp-image-7428 lazyload\" width=\"512\" height=\"342\"\/><\/noscript><\/figure>\n<\/div>\n\n\n<h2 class=\"wp-block-heading\">Is it safe for people with IBD to receive the COVID-19 Vaccination?&nbsp;<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">For patients with <a href=\"https:\/\/cdhf.ca\/en\/digestive-conditions\/ibd-crohns-disease\">IBD<\/a>&nbsp;<strong>not on immunosuppressive therapy,<\/strong>&nbsp;the Canadian Association of Gastroenterology (CAG)&nbsp;<em>recommends<\/em>&nbsp;the COVID-19 vaccine be given (strong recommendation, moderate-certainty of evidence).<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">For patients with IBD&nbsp;<strong>on immunosuppressive therapy,&nbsp;<\/strong>CAG&nbsp;<em>suggests&nbsp;<\/em>the COVID-19 vaccine be given (conditional recommendation, low-certainty of evidence).<\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><strong>What does a vaccine recommendation mean according to the Canadian Association of Gastroenterology?<\/strong><\/h2>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>A strong recommendation:<\/strong>&nbsp;CAG is confident that the benefits of following the recommendation clearly outweigh the harms, so the course of action should apply to most patients.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Conditional recommendation:&nbsp;<\/strong>means that the desirable effects of adherence to a recommendation probably outweigh the undesirable effects, but CAG is not confident that these tradeoffs due to low or very low certainty of the evidence, uncertainty regarding the balance of benefits and harms, uncertainty in patient\u2019s values and preferences or questionable cost-effectiveness.&nbsp;<strong>Therefore, conditional recommendations require shared decision-making as different choices will be appropriate for different patients.<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>**&nbsp;<\/strong>CAG mentions it\u2019s important to note that there is no standard definition of immunosuppression. Patients with IBD who are not considered immunosuppressed at diagnosis, but subsequently may become immunocompromised due to IBD medications.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">This will be published in the Journal of the Canadian Association of Gastroenterology.
