It affects about 1 in 100 people worldwide, though roughly two-thirds of people remain undiagnosed. When someone with <a href=\"https:\/\/cdhf.ca\/en\/digestive-conditions\/celiac-disease\/\">celiac disease<\/a> eats gluten, it triggers an immune response that damages the small intestine. Over time, this damage can lead to malabsorption of nutrients, causing various health issues. [1-2]<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">There is a hereditary component to celiac disease. Those with a first-degree relative have an increased risk of developing the condition. In addition, celiac disease can manifest at any age and predisposes patients to an increased risk of other pathologies, including coronary artery disease, small bowel cancers, osteoporosis, epilepsy, obstetric complications and other autoimmune conditions such as type 1 diabetes mellitus and multiple sclerosis. [3] In this article we will discuss testing for celiac disease, and why it&#8217;s important. <\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><strong>Common Symptoms and Why Testing is Important<\/strong><\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Symptoms vary widely, and some people might not have any noticeable symptoms at all. Up to 40% of people may have &#8220;silent&#8221; or non-obvious symptoms. Without testing, celiac disease can go undiagnosed for an average of 6 to 10 years, leading to irreversible damage.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Notable symptoms of celiac disease include [4-6]:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Digestive problems<\/strong> (bloating, diarrhea, constipation)\u2014reported by 70% of people with celiac disease.<\/li>\n\n\n\n<li><strong>Fatigue and weakness<\/strong>\u2014affects more than 50% of adults with the condition.<\/li>\n\n\n\n<li><strong>Unexplained weight loss<\/strong>\u2014experienced by around 30% of those diagnosed.<\/li>\n\n\n\n<li><strong>Anemia (low iron levels)<\/strong>\u2014present in nearly 50% of newly diagnosed cases.<\/li>\n\n\n\n<li><strong>Bone or joint pain<\/strong>\u2014experienced by 30-40% of people.<\/li>\n\n\n\n<li><strong>Skin rash (dermatitis herpetiformis)<\/strong>\u2014affects about 15-25% of those with celiac disease.<\/li>\n\n\n\n<li><strong>In children<\/strong>: Delayed growth or puberty\u2014common in up to 10% of pediatric cases.<\/li>\n<\/ul>\n\n\n\n<figure class=\"wp-block-image size-large\"><img decoding=\"async\" src=\"data:image\/gif;base64,R0lGODlhAQABAIAAAAAAAP\/\/\/yH5BAEAAAAALAAAAAABAAEAAAIBRAA7\" data-src=\"https:\/\/cdhf.ca\/wp-content\/uploads\/2024\/08\/Banners-for-Website-Size-86-1024x461.webp\" alt=\"testing for celiac disease villi \" class=\"wp-image-57689 lazyload\"\/><noscript><img decoding=\"async\" src=\"https:\/\/cdhf.ca\/wp-content\/uploads\/2024\/08\/Banners-for-Website-Size-86-1024x461.webp\" alt=\"testing for celiac disease villi \" class=\"wp-image-57689 lazyload\"\/><\/noscript><\/figure>\n\n\n\n<h2 class=\"wp-block-heading\"><strong>Who Should Get Tested?<\/strong><\/h2>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>First-degree relatives<\/strong> of someone diagnosed with celiac disease (parents, siblings, children) have a <strong>1 in 10 chance<\/strong> of developing the condition.<\/li>\n\n\n\n<li><strong>Individuals with type 1 diabetes<\/strong> or other autoimmune disorders are at higher risk.<\/li>\n\n\n\n<li>People experiencing persistent digestive issues, unexplained fatigue, or symptoms listed above should also consider testing.<\/li>\n\n\n\n<li><strong>People with unexplained infertility, anemia, or early-onset osteoporosis<\/strong> might benefit from testing. This includes people with Down syndrome, Turner syndrome, Williams syndrome, and selective immunoglobulin A (IgA) deficiency. [5-6]<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">If you are already following a <a href=\"https:\/\/cdhf.ca\/en\/tag\/gluten-free\/\">gluten-free diet<\/a>, you should get tested before starting, as gluten needs to be in your system for the tests to be accurate. [6]<\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><strong>Types of Celiac Disease Tests<\/strong><\/h2>\n\n\n\n<h3 class=\"wp-block-heading\"><strong>Blood Tests<\/strong><\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">This involves a simple blood draw, usually at your doctor\u2019s office.<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>tTG-IgA (tissue transglutaminase antibody test)<\/strong>: The most common initial test. It has a sensitivity rate of over <strong>95%<\/strong> for detecting celiac disease. [6-8]<\/li>\n\n\n\n<li><strong>EMA (endomysial antibody test)<\/strong>: Another highly specific test with an accuracy rate of <strong>98-100%<\/strong>. [6-8]<\/li>\n\n\n\n<li><strong>Total IgA<\/strong>: Ensures your immune system is producing enough antibodies for accurate results. Around <strong>3-5%<\/strong> of people with celiac disease have an IgA deficiency, which can lead to false negatives. [6-8]<\/li>\n<\/ul>\n\n\n\n<h3 class=\"wp-block-heading\">Genetic Testing <\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">This involves a blood test or cheek swab to collect DNA.<\/p>\n\n\n\n<ul start=\"2\" class=\"wp-block-list\">\n<li>Looks for specific genes (HLA-DQ2 and HLA-DQ8) associated with celiac disease. Over <strong>95%<\/strong> of those with celiac disease carry one of these genes. [6-8]<\/li>\n\n\n\n<li>Having these genes doesn\u2019t mean you have celiac disease, but without them, celiac is very unlikely.<\/li>\n<\/ul>\n\n\n\n<h3 class=\"wp-block-heading\">Biopsy<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">This is performed during an endoscopy\u2014a procedure where a thin tube with a camera is passed through your mouth into your intestines.<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>A small sample of the small intestine is taken (via endoscopy) to check for damage to the intestinal lining. This confirms the diagnosis if blood tests are positive.<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">If you suspect you may have celiac disease, speak with your doctor about testing.&nbsp;<\/p>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<p class=\"wp-block-paragraph\">References<\/p>\n\n\n\n<p class=\"has-small-font-size wp-block-paragraph\"><em>1.
